  • Teams of two (Achievement Course only).
  • All registration forms for a team must be submitted as a group.
  • The team captain should verify that all participants have signed their registration form.
  • Team packets must be postmarked by May 25, 2007 in order to be processed as a team.

By submitting this form, I understand that all participants, including those under 18, consent to the provisions of the CityQuest Challenge activities. I consent each participant is voluntary in this event and in good physical condition. I hereby assume full and complete responsibility for any injury or accident which may occur during our participation in this event or while on the premises of this event, and each participant hereby releases and holds harmless and covenant not to file suit against Junior Achievement, City of Reno, City of Sparks, its local Affiliates and any affiliated individuals, the Regional Transit Authority and any affiliated individuals, any City Quest Challenge sponsors and their employees, and all other persons or entities associated with this event (the “releasees”) from any loss, liability, damage or claims we may have arising out of our participation in this event, including personal injury or damage suffered by me or others, whether same be caused by falls, contact with other participants, negligence of the releasees or otherwise. If I do not follow all the rules of this event, we understand that we may be removed from the competition. We give our full permission to Junior Achievement and its local Affiliates and their sponsors to use any photographs, videotapes, or other recordings of me that are made during the course of this event.
